Step 2: Pick 1 or 2 tapes from GLITTER, HOLOGRAPHIC or SHINE tape. ( Pick 2 tapes out of those categories; You can mix the types of tapes; Please only pick 2 total...not 2 from each category. )
Step 3: Make sure you bought a "Custom Hoop" before you fill out this form; And fill in your first and last name on this form.
